Adults ages 19 through 64 receiving Medi-Cal through the Affordable Care Act expansion must work or volunteer at least 80 hours per month starting in 2027. This mandate applies to the New Adult Group under federal law H.R. 1. These individuals face stricter eligibility checks and new activity requirements.

The California Department of Health Care Services identifies the specific members affected. Not every Medi-Cal participant falls under these new rules. The department sends notices to those who must meet the Work and Community Engagement Requirements. San Bernardino County will assist members in determining their status.

New applicants will face evaluation for these requirements beginning January 1, 2027. Current members receive notices in March 2027. These letters explain specific obligations and request additional information if needed. The six-month renewal cycle also begins in March 2027 for affected groups.

Acceptable activities to meet the 80-hour threshold include paid or self-employment, volunteering, job training, and half-time school attendance. A monthly income of at least $580 also satisfies the requirement. Individuals may combine different activities to reach the total hours. Seasonal workers or those with fluctuating incomes may have additional options.

Certain members qualify for exemptions from the work requirement. These include pregnant individuals, parents of children 13 or younger, and people with disabilities or medical conditions that prevent compliance. Individuals with Medicare, American Indian or Alaska Native status, and former foster youth under 26 are also exempt. San Bernardino County reviews available data to confirm these statuses.

Eligibility reviews change from an annual to a six-month schedule for affected members. A member completing a renewal in March 2027 faces another review in September 2027. Being excused from work requirements does not automatically exempt members from these frequent reviews. Other coverage groups maintain their existing annual schedules.

The California Department of Health Care Services launched a text message campaign in February 2026. Mailed outreach letters reached current New Adult Group members in August 2026. San Bernardino County includes this information in application materials through the end of 2026. Letters are available in multiple languages to assist diverse populations.

Members must keep their contact information current with the county. They should check mail and online accounts regularly for notices. Renewal forms and requested documents require submission by stated deadlines. The BenefitsCal online platform allows users to manage benefits and upload documents. San Bernardino County coordinates with community partners and federal agencies to share updates.
